Review financial, legal, or medical issues and offer referrals to specialists to avoid future problems and conserve assets
Provide financial and daily business management and oversight, powers of attorney, conservatorship or guardianship assistance
Conduct Intakes and Assessments to identify problems, eligibility for assistance, and need for services
Screen, arrange, and monitor in-home help or other services
Act as a liaison to families at a distance, making sure things are going well, and update families to problems or concerns
Assist with maintaining independence in one’s current housing situation
Coordinate moving an older person to alternative housing, such as from one’s home to a retirement complex, care home, assisted living, or long-term care facility
Offer support during times of crisis where a one-to-one intervention is necessary for your loved one’s health.
